You’ll be able to confirm your Smilist ONE Membership details before enrolling.
There are five Smilist ONE Membership plans to choose from. You can choose to pay your membership fee annually or monthly. By choosing to pay annually, you’ll receive one free month of the Smilist ONE Membership Program. The pricing for the annual membership reflects the one free month.
- Individual Plan – $30/Month or $330/Year
- Couple Plan – $55/Month or $605/Year

No, the Smilist ONE Membership Program is not a dental insurance plan and not intended to be a substitute for a dental insurance plan. Rather, the Smilist ONE Membership Program is an affordable dental membership program that allows you to receive valid dental services at any affiliated Smilist Dental office.
The Smilist ONE Membership Program is valid at many of our Dental offices. Some restrictions may apply. Email patientcare@thesmilist.com if you have any questions regarding office participation.

To book an appointment with your Smilist ONE Membership, you can book your appointment online or call our office. When booking your appointment online, select “Smilist ONE Membership” under the list of insurances. If calling our office, state that you are a Smilist ONE member. Our office will confirm your appointment with you.
To cancel your Smilist ONE Membership, you can (1) come into one of our Smilist Dental locations and talk to a member of the patient care team or (2) mail your statement of cancellation of Smilist ONE Membership to the Smilist Dental office you are a patient of. If the Smilist ONE Membership is cancelled before the end of the 1 year, the participant is responsible for the remaining balance. After the 1 year commitment, the Membership can be cancelled at any time. The credit card on file will be automatically charged monthly or annually, depending on the Membership selected, until cancelled.
The Smilist ONE Membership is non-transferrable and no refunds will be issued if participant decides not to, or is unable to, utilize the Membership, or for any reason. After the 1 year commitment, the Membership can be cancelled at any time. If you chose the monthly charge, The Smilist Dental will no longer charge the credit card on file once you cancel your Smilist ONE Membership. If you chose the annual charge, The Smilist Dental will refund the balance for the year less the months that your membership was active.
The Smilist ONE Membership is NOT A DENTAL INSURANCE PLAN and is not intended to be a substitute for a dental insurance plan. There is a one-time, non-refundable $50 enrollment fee and a 1-year commitment per Smilist ONE Membership and if Membership is canceled before the end of the 1 year, the participant is responsible for the remaining balance. After the 1 year commitment, the Membership can be canceled at any time. The credit card on file will be automatically charged monthly or annually, depending on the Membership selected, until cancelled. The Membership is non-transferrable and no refunds will be issued if the participant decides not to or is unable to, utilize the Membership, or for any reason. The Smilist ONE Membership is only valid for services rendered at an affiliated participating Smilist Dental office and cannot be used anywhere else.
